Thank you for considering Wings Trust.

Assessment forms are available on request from the Wings Administrator officer. Please phone (09) 815-1631 extn 0 and leave a message if the phone is not answered.

In order to ensure that your referral is processed as efficiently as possible it is important that wherever possible a counsellor / GP / social worker completes this assessment form.

All clients are expected to be at least 7 days free from alcohol and other drugs other than prescribed medication. We reserve the right to refuse admission.

Current medication will be represcribed by the Wings Trust doctor following entry to the service. 

Before your client is able to enter Wings the following matters need to be addressed. This is to save the client any possible hardship at a personal and financial level.

• Beneficiary status is documented.  Please inform the client that they will need to complete a sickness benefit benefit application.     
• Any current debt being recovered from the clients benefit must not exceed $40.00 per week.Please include child support where applicable. 
   
• Current medication is documented. Reduction regimes must be documented.
  
• Special dietary needs are documented 
  
• Specialist treatments which are ongoing.
• A discharge summary (Medical and social detox)
• A treatment summary for those exiting treatment programmes

We provide a range of group work four mornings per week. Clients are allocated individual counsellors. We have an initial minimum requirement of clients attending 5 twelve-step meetings per week
